St. Clare Health Mission employs several individuals who are actively involved with the follow-up care of our patients. These individuals are called community health workers who are tailored to meet the unique needs of the communities they serve.
CHWs hold the following roles:
Creating connections/appointments with healthcare systems
Connecting clients with basic needs such as housing and food
Helping to manage care
Determining eligibility and enrolling individuals into health insurance plans
Providing culturally appropriate health education on topics related to chronic disease prevention, physical activity and nutrition
Advocating for clients
Providing informal counseling, health screenings, and referrals
Address health issues
Outreach and enrollment
Support care delivery
Social-emotional support
